The Membership process is extremely easy:
- Determine all eligible employees for membership - uninsured, part-time and current high deductible plan employees. Distribute education and election/waiver forms to complete.
- Provide Christ Clinic with a listing of those employees electing or declining membership.
- Based on enrollment, monthly contribution will be $40 per employee, $40 per spouse, and $30 per child.
- Monthly adjustments will be a streamlined listing of participants to be adjusted for additions and deletions.
- As employees utilize the clinic, they only pay $5 per visit, will receive highly discounted Prescription Drugs, and huge discounts for Lab Services (usually 80%+).
