What is 2535 initiative?

2535 is a church-clinic under the care of Christ Clinic that aims to provide medical assistance to community members in need. Our goal is to reach out to all individuals who need access to medical care in Houston despite of any barriers that may have prevented you from getting help in the past. 


Our clinics will provide medical professionals to diagnose and treat patients. We also have a strong support system through our loving staff.

• Screenings
• Preventative Care
• Chronic Disease Management

Progress

Christ Clinic is currently working with ten multiple partners to implement ten sites in quarter two for the 2535 Initiative and will use this discovery period to identify challenges and streamline solutions for future partners.

  • Texas Association of Charitable Clinics
  • Christian Community Health Fellowship
  • 75 local churches
  • ~175 volunteers

Goals

2535 will open 30 church-based clinics by the end of 2023.

  • Each church will initially open a 2535 clinic location and provide clinic days once per month
  • Based on demand, expect to increase to a minimum of twice  monthly by the end of 2023 per location.
  • 18,000 additional patient visits will be provided through 2535 30 churches will invest in social services for 2535 patients to address issues such as: 

                     -Food Assistance

                     -Transportation

                     -Job Readiness, Connections & Networking

                     -Housing

                     -Crisis Management

                     -Healthy Living and Disease Management Classes

                     -Community Support Groups

                     - Financial planning
