If you’re uninsured and your income is below 200% of the Federal Poverty Guideline, you could qualify for tailored primary, preventative, and specialty care. Patient engagement and regular compliance checks are required.

Christ Clinic’s behavioral health model addresses mental illness in our patient population. Services include mental health evaluations, limited psych medications and short-term therapy.

To be eligible for our Total Care Program, patients must meet certain requirements, including:

Patients must fill out the Patient Contract Agreement, have all paperwork on file, and present their Total Care ID at appointment.
